Electronic Records and Signature Consent

What you are agreeing to
You have been sent a document to review and sign electronically through Acutance, a service operated by CHNH, LLC (“we,” “us”). By checking the consent box and proceeding, you agree to:
- Transact electronically. You consent to receive, review, and sign this document — and to receive related records and notices — electronically rather than on paper.
- Sign electronically. You agree that typing your name and completing the signing action constitutes your legal signature, with the same effect as a handwritten signature on paper.
Your consent applies to this document and the records connected with it. The person or business that sent you the document — not CHNH — is the other party to it; we provide the signing service.
If you sign on behalf of a business or another person, you represent that you are authorized to sign for and bind them.
The Service is not intended for records excluded from electronic-signature laws — for example wills and testamentary instruments, certain family-law documents, court papers, and statutory notices such as those concerning foreclosure, eviction, utility shutoff, insurance cancellation, or product recalls. Do not sign such records through the Service.
What we record when you sign
To make your signature attributable and verifiable, we record an audit trail with the signed document: your typed name and email address, the date and time, your IP address and browser information, the consent language you agreed to, and a cryptographic fingerprint (hash) of the document exactly as it existed when you signed. This information is retained with the signed document and may be provided to the parties to the document as evidence of signing. Our Privacy Policy describes this in more detail.
Your right to a paper copy
After signing, you may download and print the completed document at no charge. You may also request a copy from the party who sent it to you. Acutance is not a permanent records custodian, and CHNH may be unable to provide a copy after the sender’s account and associated records have been deleted. Download and retain your copy when it is made available.
Keeping a copy
After signing, you will be able to download a PDF of the completed document, and a copy is available at the same secure link you used to sign. Download and retain the completed document and any accompanying audit certificate. Acutance is not intended to serve as your permanent repository, and continued access may end if the sender’s account is closed or the Service is discontinued.
Withdrawing consent
You may decline to sign electronically. Before signing, you can simply close the page or use the decline option, and then contact the sender to arrange a paper alternative — declining electronic signing does not by itself reject the underlying agreement, it only means it won’t be completed through this service. Withdrawing consent after you have signed does not undo the signature already given. To withdraw consent to future electronic records, notify the sender or email legal@chnh.llc.
Keeping your contact information current
The email address associated with your signature is how the parties and we can reach you about the signed record. If it changes, notify the party who sent you the document, or email legal@chnh.llc with your old and new addresses.
Hardware and software requirements
To sign electronically you need: a device with a current web browser (supporting TLS 1.2 or later and JavaScript), an internet connection, a valid email address, and the ability to view and store PDF files. If these requirements change in a way that creates a material risk you cannot access your records, we will notify you at the email address associated with your signature.
